HOME SECURITY CHECK FORM: If you are planning a trip or will be away from your residence for an extended period of time, you can complete this form requesting a house check. The information contained in this form will help the police department if you should need to be contacted in an emergency. It also allows you to designate an alternate contact or key holder we can contact if needed. Officers will also check on your residence periodically.

VENDOR PERMIT APPLICATION: Organizations, businesses and individuals wishing to sell merchandise or solicit funds door to door or in temporary locations within the boroughs of Lemoyne and Wormleysburg, must first complete a vendor permit application. The completed application must then be brought to the police department for approval and  the Chief of Police or his designee will issue the permit. Religious groups and non-profit organizations do not need permits.
